Appointment Cancellations

·      courtesy reminder calls provided 48 hours in advance of appointment

·      24 hour notice required for cancellation

·      $100 charge billed for office visit no show or cancel less than 24 hours

·      $300 charge billed for surgery no show or cancel less than 24 hours

Product Returns

·      all returns must be unopened and in original packaging

·      products may be returned within 15 days for a 20% restocking fee

·      after 15 to 30 days products may be returned for in office credit only

·      beyond 30 days products are nonreturnable and nonrefundable
