Appointment Cancellations

  • Courtesy reminder calls provided 48 hours in advance of appointment
  • 24 hour notice required for cancellation
  • $75 charge billed for new patient office visit appointment no show or cancel less than 24 hours
  • $50 charge billed for follow up patient office visit appointment no show or cancel less than 24 hours
  • $150 charge billed for surgery or cosmetic appointment no show or cancel less than 24 hours

 

Purchase Policy Custom Medication Products

  • Prescription medications sold in our office are nonrefundable due to quality control
  • We no longer accept prepayments for prescription products
  • You must be present to purchase a prescription product

 

Refund and Exchange Policy for Cosmetic Retail Products

  • Retail products must be in their original packaging & unopened for exchange/refund within 15 days with a 20% restocking fee 
  • After 15-30 days we will exchange a product the same day as you bring the item into the office, but no longer offer a credit
  • Beyond 30 days products are nonrefundable and nonreturnable
  • Products must be exchanged for other products the same day  
  • The exchange of products is not valid towards cosmetic treatments 
  • Special orders with shipping costs must be paid for at the time of the order & we will notify you when the item(s) arrives; if after 2 weeks we do not hear back or if the item is not picked up, then we will put the item(s) back on the shelf

  • Copays  
  • Outstanding Balances
